Ongoing Service Coordination helps adults with developmental disabilities navigate life’s complexities while maintaining their independence and achieving their personal goals. Our Service Coordinators take a lifelong, person-centred approach, ensuring that individuals have the support they need to fully participate in their communities, build meaningful relationships, and live fulfilling lives. We meet individuals where they are—whether at home, in the community, or virtually—working together to create sustainable, person-driven solutions.

We Support With:

  • Personal Goal Planning: Helping individuals identify their goals and aspirations, and providing support to achieve them.
  • Food Security & Nutrition: Assisting with access to healthy food options, meal planning, and nutrition education.
  • Transportation Skills: Supporting individuals in learning how to navigate public transit and transportation options safely and confidently.
  • Navigating Legal & Medical Systems: Providing guidance and advocacy to help individuals understand and access their rights and healthcare needs.
  • Accessing Services: Helping individuals connect with community programs and resources that best meet their needs.
  • Employment Readiness & Workplace Support: Supporting individuals to develop the social and practical skills needed for employment, assisting with job searches, and providing ongoing workplace support when needed.
  • Housing & Independent Living: Assisting with securing safe and appropriate housing and developing independent living skills.
  • Community Participation: Encouraging social inclusion through engagement in community activities, building meaningful relationships, and fostering belonging.

Ongoing Service Coordination is available to adults with developmental disabilities who require consistent, long-term support to navigate daily life and achieve their goals while working towards, or living independently in the community.

We provide support in Brantford, Brant County, Six Nations of the Grand River, and the Mississaugas of the Credit First Nation. Services are offered in homes, community spaces, and virtually to ensure accessibility.

Adults can access service through Developmental Services Ontario (DSO) at 1-877-376-4674.

Ongoing Service Coordination is a free service for eligible individuals registered with DSO.
