About

Becoming an adult is an exciting time, but it can also feel overwhelming. Our Transitioning to Adulthood (TAY) program supports youth living with an intellectual disability in Extended Society Care as they plan for their future. We work alongside youth, caregivers, and community partners to explore opportunities, build independence, and create a Transition Plan that reflects their dreams and goals.

Through collaboration with child and adult service providers—including Children’s Aid societies—we ensure a smooth transition into adulthood, helping youth take confident steps toward independence.

Things We Do:

  • Future Planning: Helping youth explore career, education, housing, and independent living options.
  • Life Skills Development: Supporting youth in learning daily living skills, budgeting, and decision-making.
  • Transitions to Adult Housing: Assisting youth in finding suitable housing options and understanding available supports.
  • Connections to Services: Linking youth to adult developmental services, employment programs, and social opportunities.
  • Advocacy & Support: Ensuring youth have a voice in their transition planning.
  • Collaboration with Child & Adult Services: Coordinating with community agencies to create a seamless transition into adulthood.

This program is available to youth in Extended Society Care in Hamilton, typically between the ages of 14–18, who are living with an intellectual disability.

Support is available in Hamilton through in-person, virtual, and community-based meetings to best meet each youth’s needs.

Referrals are made directly by:

  • Children’s Aid Society of Hamilton
  • Catholic Children’s Aid Society of Hamilton
