Enhanced Service Coordination provides short-term, intensive support for adults with developmental disabilities facing urgent and complex challenges. Our Service Coordinators work across sectors—including healthcare, social services, and housing—to stabilize crises and help individuals access the right supports. We meet people where they are, responding to their unique needs with respect and dignity.

Some Concerns
We Support With:
- Crisis Stabilization: Immediate support for individuals at serious risk of homelessness, food insecurity, or safety concerns.
- Cross-Sector Collaboration: Working with healthcare, social services, and legal supports to provide coordinated care.
- Housing Security: Helping individuals access emergency shelter and secure housing.
- Mental Health & Trauma Support: Connecting individuals with appropriate mental health resources and trauma-informed care.
- System Navigation: Assisting with accessing healthcare, social services, and community supports.
- Personalized Safety Planning: Developing plans to address immediate personal safety.
- Care Coordination for Medical Needs: Supporting individuals with complex health conditions to access the care they need.
- Legal & Financial Advocacy: Helping individuals navigate legal and financial systems to achieve stability.
Enhanced Service Coordination is for adults (18+) with a developmental disability who are also experiencing:
- A dual diagnosis (mental health and developmental disability)
- A serious medical condition
- Past trauma
- Complex life situations requiring individualized support
Services are available in Brantford-Brant, Hamilton, Niagara, Haldimand-Norfolk, Six Nations of the Grand River, Mississaugas of the Credit First Nation. Support is offered in homes, community spaces, and virtually.

Enhanced Service Coordination is a free service for eligible individuals registered with DSO.
