EnCompass Service Coordination
EnCompass Adults
Our service coordinators offer adults living with an intellectual disability the support they need to access safe housing, medical, financial, or mental health services and to develop socially valued roles within their community.
EnCompass Adults supports individuals to develop important life skills, such as budgeting, nutrition, travelling via public transit, and to learn their rights and responsibilities. When making connections becomes difficult, our Service Coordinators go the extra mile and head out into the community to meet clients where they are in their moment of need. We keep in touch, stay accessible, and work with community members to reach their goals.
Ongoing Service Coordination
Ongoing Service Coordination is designed to provide long-term support to adults 18+ living with a developmental disability in the city of Brantford and Brant County. Our program captures all aspects of advocacy, skill-building, goal setting, formal and informal community participation and service coordination for individuals seeking to live more independent lives.
Enhanced Service Coordination
Enhanced Service Coordination is designed to provide short-term, intensive support to adults 18+ with a developmental disability and dual diagnosis, serious medical condition, experienced trauma or a complex situation requiring more individualized and concentrated support. We ensure that individuals receive the help they need to stabilize critical situations and find support within the community. Often, this means working and collaborating with a variety of services including mental health, corrections, health, and developmental services to stabilize individuals and ensure their safety.
This service is available to residents of Brantford, Brant County, Haldimand Norfolk, Niagara and the Hamilton regions, as well as First Nation communities of the Six Nations of the Grand River and Mississaugas of the New Credit First Nation.
